Central Middle School Health Services

StudentWelcome to the Middle School. The school's environment is quite different from the elementary schools. The size of the building, the number of students and staff members, and the team concept are all new to many students.

I understand that the middle school years are exciting, exhausting, and at times very challenging. Certain issues that may arise that are inherent to this age may be perplexing and confusing. 

The nurse at Branchburg Central Middle School performs the state-mandated screening programs, offers student support, and provides nursing care during these early adolescent years. The goal is for a healthy, successful school year.

Interscholastic and Intramural Sports Physicals

State law requires that a current, cleared PHYSICAL EXAMINATION form be on file in the health office for a student wishing to try-out/participate in a specific interscholastic or intramural sport. The physical examination shall be conducted within 365 days prior to the first practice session (try-out session). It is highly recommended that your primary care physician complete the Physical Examination, however the student's parent/guardian may choose either the child's own private physician or the school physician to perform the physical examination. To make arrangements for a sport physical examination by the school physicians, please call Hunterdon Family Medicine at Bridgewater at 908-237-4135.

For each individual sport season (Fall, Winter, Spring) your child wishes to try out for, ONLY a new, current HISTORY form must be completed by the parent/guardian.

The deadline to turn in medical forms in order to try out for a Fall Sport is during August (please check Athletics Webpage for exact dates).  Paperwork will NOT be accepted after the due date. If paperwork is not in by the due date, the student will not be eligible to participate in athletics.

BCMS ATHLETICS RULES

  1. Turn in signed permission slip to coach by due date
  2. Turn in completed physicals to Nurse by due date

History and Physical Examination Forms must be entirely completed (NO BLANK SPACES) by student’s parents and medical provider and then signed off by the Branchburg School Physicians in order for participation in BCMS Athletics. 

Any forms handed in after the deadline will not be approved and your student will not be able to participate in the sport.

NO EXCEPTIONS WILL BE MADE TO THESE REGULATIONS.

Please Note:

Sports physicals must be received by the school nurse on or before the physical deadline as listed on the Athletics Webpage. Please review the completed physical form to insure that all medical information has been provided. Incomplete forms, with areas left blank, will be returned to you for your follow up with your physician. 

State law requires that the physicals are reviewed by the school physician.

Helpful Health Tip



Make a copy of all your child’s health-related forms and store them in your child’s medical file at home. This should include your child’s completed physical examination form, physician reports (primary care physician and specialists), physical education excuses written by your child’s physician, dental reports, and any other documents you receive regarding the health of your child.
